Allegations
Whilst registered as a Social Worker (SW9066) when working for Bradford Children’s Social Care (as it then was):
1. On or around 2 March 2021:
a. You did not call Service User A within 24 hours of being assigned the case.
b. You recorded on Service User A’s record that you had called and spoken to Service User A when you had not done so.
2. On or around 2 March 2021 you failed to safeguard Service User A and/or their child as:
a. You did not agree a safety plan with Service User A within the appropriate timeframe.
b. You recorded on Service User A’s record that a safety plan had been agreed with Service User A when it had not been.
3. On or around 4 March 2021:
a. You told Colleague A that you had called Service User A when you had not done so.
b. You told Colleague A that you had agreed a safety plan with Service User A when you had not done so.
c. You provided telephone records to Colleague A showing two calls had purportedly been made to Service User A, but these were calls to a 10 digit invalid number.
4. Your actions at paragraphs 1b, 2b, and 3 were dishonest.
The matters set out at paragraphs 1, 2, 3 and 4 amount to the statutory ground of misconduct.
By reason of your misconduct your fitness to practice is impaired.
