Hearings and decisions

Whilst registered as a social worker between around September 2021 and April 2022, you: 

1. Failed to adequately safeguard service users in that you: 

a. Self-authorised the closure of approximately 111 Safeguarding Concerns and / or Safeguarding Enquiries without management oversight 

b. Failed to adequately investigate safeguarding concerns in respect of one or more of the following self-authorised Safeguarding Enquiries: 

i. Service User A 

ii. Service User B 

iii. Service User G  

iv. Service User F 

c. Failed to obtain and / or record relevant and / or accurate information within the Safeguarding Enquiries for one or more of the following: 

i. Service User C

ii. Service User D

iii. Service User E 

iv. Service User F 

d. Recorded within the self-authorised Safeguarding Enquiry that a face to face meeting had taken place with Service User F and / or Service User F’s husband on 22 April 2022, when this was not the case.

e. Recorded within the self-authorised Safeguarding Enquiry that feedback had been given to the adult and/or advocate for service user A when this was not the case.

f. Recorded within the self-authorised Safeguarding Enquiry for Service User G that you had visited the Service User when this was not the case. 

2. Your actions at paragraphs 1(d) and / or 1(e) and / or 1(f) were dishonest in that you recorded information which you knew to be false.
