Allegations
1. When you attempted a visit to Family A on 29 May 2019, you:
a) Did not attempt to make contact with the mother when you found she was not at home;
b) Asked her son to pass on the message that you had visited.
2. You did not communicate effectively with Family B in that you:
a) Did not attend a visit on 10 June 2019 to Family B as arranged;
b) Did not make contact with Family B to explain that you would not attend the visit;
c) Did not contact Family B again until 14 June 2019.
3. While you were the allocated social worker for Family C, you did not:
a) Attend a Network meeting at Service Users CA and CB’s school scheduled to take place on 24 June 2019;
b) You did not contact the school to inform them that you would not be in attendance at the meeting.
4. Between 2 September 2019 and 4 September 2019 when asked to remain with Service User D to facilitate a handover to their father, you:
a) Did not follow management instruction to remain with the service user until their father arrived to collect them;
b) Left Service User D’s home to attend a visit with another family;
c) Did not inform your manager and/or Service User D’s father that you had left;
d) Were not present when the father arrived and/or did not answer his call;
e) Were not contactable when your manager tried to contact you;
f) Asked the parent of the second family for a lift back to Service User D’s home.
5. While you were the allocated social worker for Service User E from around 26 February to 30 April 2020:
a) In a Child in Need meeting on 24 April 2020, you did not chair the meeting effectively in that you:
i. Did not circulate the Child in Need plan in advance of or during the meeting;
ii. Did not discuss the Child in Need plan and/or the outcomes during the meeting;
iii. Had not progressed the action points since the previous meeting on 20 February 2020;
iv. Did not enquire about Service User E’s living arrangements.
b) During a Child in Need meeting on 24 April 2020, you indicated that you did not know what a personal budget was, despite having reported on 27 March that you had made a request for one.
c) In an email on 23 April 2020, you requested information to enable an Education Health and Care needs assessment to be completed, despite Service User E having already been awarded an Education Health and Care Plan.
d) In your record of a home visit with Service User E on 16 March 2020, you did not provide analysis of their mother’s mental health concerns.
6. While the allocated social worker for Service User F:
a) From 12 March to 5 April 2020, you did not follow management guidance from AB to:
i. Prepare a grab pack;
ii. Conduct a return to home interview in an appropriate setting;
b) Your case note dated 11 March 2020 was not clear and/or did not include adequate analysis of risks;
c) You did not complete a case note on 12 May 2020 regarding contact with the safeguarding officer.
7. In the following Child and Family assessments, your written communication was not clear and/or lacked analysis:
a) Child and Family Assessment for Family G dated 16 August 2019
b) Outreach form for Family G
c) Child and Family Assessment for review child protection case conference, completed on 14 April 2020 for Family H
d) Child and Family Assessment for review child protection case conference, completed on 19 July 2019 for Service User D
e) Child and Family Assessment completed on 23 March 2020 for Family I
f) Child and Family Assessment for review child protection case conference, completed on 28 August 2019 for Service User J
8. Whilst you were the allocated social worker for Service User X from 7 February to 8 September 2020, you:
a) You took your child with you to one or more child protection visit(s) with Service User X on a date or dates unknown;
b) Took and/or allowed Service User X’s mother to take one or more photos of Service User X with your own child;
c) Your actions at a and/or b above represent a failure to maintain professional boundaries
The matters outlined at 1 to 7 above amount to the statutory ground of lack of competence or capability and/ or misconduct.
The matter outlined at 8 above amounts to the statutory ground of misconduct
By reason of your misconduct and/or lack of competence or capability, your fitness to practise is impaired.
