Whilst employed as a Youth Offending Service Officer as a registered social worker at Buckinghamshire County Council:
1. You failed to maintain full and accurate records for service users A-K between December 2018 and May 2019, in that you failed to record your contacts promptly, or at all, with Service Users A-K as set out in Schedule 1.
2. You failed to remedy managerial concerns raised with you about your lack of contact recording in respect of Service Users A-K as set out in Schedule 2.
The matters outlined in paragraphs (1) and (2) amount to the statutory ground of misconduct and / or lack of competence / capability.
Your fitness to practise is impaired by reason of misconduct and / or lack of competence / capability.
